Global Form Settings:
Form Field Labels: This gives the formatting of the text for the form headings.
Thank You Message: Thank You For contacting us. We will get back to you as soon as possible. <br><Br><Br>Please resubmit the form if you want to add more comments or else use the left hand buttons in order to visit the rest of the site. This is the thank you message used after the form is submitted.

Either or, or or etc. type questions - several choices - tick only one

Add Options

This gives the options for the radio buttons

Checkbox
Tick this option if you agree type questions.

Text entry fields

Text field - Characters and lines
Text entry fields with multiple lines

Width:
No. of lines:

Display Order
The display order of the question in the list - 1 = top etc..

Required
Is the question mandatory

Adds a question, based upon the criteria set above.

When you have added a question, it is then listed below and you can add, edit (they appear above for you to edit) delete, change display order and make it mandatory or not.
